Not enough data to really help. Are you listening through some external speaker, maybe Bluetooth speaker or headset? If so, disconnect that speaker or headset and listen a sample through the internal speaker of the phone or tablet you are using. Is it correct now? If so, your external speaker is programmed to "fade in music" (each new song starting from silence and gradually increasing in volume). Read the manual or ask the speaker/headset manufacturer for support, how to disable this "fade in" feature.
 
Another possibility would be if you defined some speech replacements under the Settings menu - Edit speech, and an entry there is incorrect, most probably an entry using RegEx. Delete all suspicious entries from the Edit speech list and test again.
